The Mine is in permanent cessation and no
Energy Fuels personnel remai n on the mine site. The weather was cloudy, and there was evidence of recent rains
in the area. The ground was damp and signs of recent runoff flow was apparent along the roadway and in the
collection ditches
HYDROLOGIC BALANCE - Rule 4.05 : Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5,
4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Impoundments 4.05.6, 4.05.9;
Pond 5 was damp but held no standing water. The western culvert outlet was full of sediment.(Photo1) Runoff
from re cent rainstorms appears to have made it into the pond through the remaining 1 inch opening at the top of
the culvert. Sediment was n o t backed up into the inlet of the culvert. The grade on this culvert is adequate to keep
sediment from settling in the mid dle of the culvert. Energy Fuels needs to clean this culvert to assure proper
function of the culvert and ditch. The eastern culvert into the pond is open both at the inlet end and the outlet.
Runoff from the ditch is able to enter the pond through the eastern culvert. Sediment in the bottom of the ditch
illustrated that recent h eavy flows had moved through the ditch and into the pond recently. Adequate storage
capacity remains in the pond. Sediment was well below the bottom of the discharge riser. There was no visible
marker to show sediment clean out elevation.

Pond 4 was da mp with no standing water, very little sediment in the bottom of the pond and no observed
problems.

PROCESSING WASTE/COAL MINE WASTE PILES – Rul e 4.10 and 4.11 :Drainage Control; Surf ace
St abilization; Placement:

The refuse disposal area continues to have a good cover of live vegetative plants, dominated by a mixture of warm
and cool season grasses. No gullies were apparent on the reclaimed slop es. Four -wing salt bush have become well
