Please continue to retain this attachment in the permit. Attachment 2.05.3(3) -14 has been restored to the Index. Item resolved c) Attachment 2.05.3(3) -15 (Pond Slope Stability Analyses) —Page 1 refers to a "Drawing 1" depicting a cross- section of the typical pond. The Division does not have a copy of "Drawing 1" in its records. Please provide a copy of this drawing with the adequacy response. With the response, a copy of Drawing I (Typical Sediment Pond Cross Section) was provided, as requested. However, the Division has additional comments related to this Attachment. i) Drawing 1, as provided, is at a scale that is difficult to read. Please consider providing a larger version of Drawing 1, perhaps using 11 "xl7" paper instead of letter -sized paper. ii) The dimensions of the pond embankment, as depicted graphically on Drawing I and as identified by X -Y coordinates in the Case Runs provided with the Attachment appear to be at odds with those described in the text. Specifically, the last paragraph of section 3- Strength Parameters Used in Slope Stability Analysis states that the pond slopes will be 2.5H.I V. In contrast, it appears that the slopes used in the stability analysis were 4H.I V. If the Division is correct in its understanding, then the factor of safety determined for the pond embankments at the mine must he reevaluated. Please review Attachment 2.05.3(3) -15 and revise as appropriate. d) Pond 011 As -Built has been returned to Attachment 2.05.3(3) -20, as was approved with TR -56. (No response is needed.) e) Pond 013 As -Built has been returned to Attachment 2.05.3(3) -21, as was approved with TR -56. (No response is needed.) f) The Pond 015 Design is located in Attachment 2.05.3(3) -22. Please add "(PR -06)" to show the revision under which it was approved, and revise the title to be simply, "Pond 015 Design ". Item resolved. g) Attachment 2.05.3(3) -23 is the Seismic Survey approved under MR -78. Please change the revised date to December 2010. The cover letter provided with the adequacy response indicates that revised page 2.05.3(3) -23 -1 was attached. This page does not appear to have been included. Please submit the revised page. h) Attachment 2.05.3(3) -24, Pond 015 As- Built, has been added to the index. Please change "(MR -81)" to "(TR -60) ". Item resolved. 2. 2.05.3 (3) Maps Index The Division appreciates the addition of this Maps index to Section 2.05.3(3). There are, however, a number of maps from the permit that have not been included in the list provided (please see August 15 PAR for the original list). Please revise the index of Maps for Section 2.05.3(3) to include each of the items listed above. The map index was revised as requested. Item resolved. 3. 2.05.3(3) Text a) Page 2.05.3(3)-2, last paragraph —the rule referenced is incorrect —please change to 4.05.2(7). Item resolved.
