The weather was dry and windy. The <br />Southfield Mine is a reclaimed underground mine. As such, no mine personnel remain on site. The inspection <br />initiated at the mine site. Mr. George Patterson and Gary Carroll met the inspector at the Energy Fuels coal <br />Company office in Florence for access to the mine records. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />Records were checked in the Energy Fuels office in Florence. George Patterson and Gary Carroll were present <br />during the records portion of the inspection. Mr. Carroll is a contact for access to the records in the event that Mr. <br />Patterson is unavailable. <br />HYDROLOGIC BALANCE -Rule 4.05 Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, <br />4.05.9; Stream Buffer Zones 4.05.18: <br />Pond 5 was dry at the time of the inspection with no evidence of any water in the pond since sediment was <br />cleaned out this winter. <br />Pond 4 was dry at the time of the inspection. <br />Pond 5 24" inlet culvert (north culvert) has a rock or other object pushing down from the upper surface. Energy <br />Fuels need to repair this culvert to assure designed flows are able to pass through the structure. the South (30" <br />dia.) culvert is clear. <br />Ditches throughout the site are in good repair. <br />Number of Partial Inspection this Fiscal Year: 5 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 2 of 6 <br />
