MANAGEMENT <br />RECEIVIND <br />MAR 16 2012. <br />DM&O of and Safety n, <br />The DRMS' letter dated March 5, 2012 transmitted its adequacy review for the referenced minor <br />revision. Following are Honeywood Coal Company's (HON) responses to the DRMS comments <br />and concerns: <br />1. In accordance with Rule 2.03.4(2), the Operator indicated in the MR the address for <br />Honeywood Coal Company is 2120 Crestmoor Road, Suite 3020, Nashville, TN 87215. <br />However, the Division checked this information with the Applicant Violator System <br />(AVS) and it indicated the address for the Honeywood Coal Company is 95 White Bridge <br />Road, No. 404, Nashville, TN 87205. Please confirm which address is correct? If the <br />address provided in the minor revision is incorrect please send a revised page with the <br />correct information. <br />HON - The correct address is 2120 Crestmoor Road, Suite 3020, Nashville, TN 87215. <br />2. The minor revision updated the language of the current approved monitoring plan. This <br />was done as a result of the Divisions preliminary review of the pending Phase III bond <br />release (SL -03). The Division preliminary review requested the Permittee to update the <br />plan to include a full suite analysis for the bedrock wells. However, the updated plan <br />submitted only committed to measuring conductivity semi - annually (Monitoring <br />Schedule #2). The applicant will need to revise page 2.05 -35 and commit to conducting a <br />fall full suite analyses for bedrock wells. <br />HON - Page 2.05 -35 was revised to add a fall full suite analyses for bedrock wells. <br />3. Well completion information was requested back in July of 1998 and again in the <br />Division's preliminary review of SL -03 and was not provided. This information is critical <br />to assessing groundwater quantity and quality impacts. Well completion information is <br />unavailable in the permit file other than a "typical" drawing for an alluvial well and a <br />bedrock well and the total depth of the wells listed next to the well location for some but <br />not all wells. Please submit well completion information. <br />
