Access Portal Collapse Repair and Equipment and Stope Rock Removal Work Plan March 2, 2012 <br />May Day No. 1 <br />1.0 INTRODUCTION <br />This Work Plan presents Wildcat Mining Corporation's ( "Wildcat ") approach to stabilizing and <br />repairing the Access Portal, removal of collapsed material (approximately 2,300 cubic yards) and <br />buried equipment (jaw crusher, cone crusher and mucker) at the Mill Portal (Phase 1), and <br />removal of approximately 5,000 cubic yards of stockpiled rock or ore from within a stope <br />accessed by the May Day No. 1 Portal (Phase 2). The May Day Idaho Mine Complex is located <br />in La Plata County approximately 15 miles west of Durango, Colorado (Figure 1). <br />Wildcat is proposing to operate the May Day Idaho Mine Complex and is in the process of <br />activating MSHA mine identification (ID) number 05- 03674. Upon ID activation, and MSHA's <br />review and DRMS approval of this Work Plan, Wildcat will implement the corrective actions <br />described herein. <br />In April 2010, a collapse of the area surrounding the May Day No. 1 Portal occurred. The <br />collapse resulted in a cave in of two separate adits accessing the May Day No. 1, including a <br />historic access portal (the "Access Portal ") and a load -out portal containing a mill (the "Mill <br />Portal "). The May Day No. 1 Affected Area consists of a relatively flat pad, a mill and a main <br />access tunnel/adit (Photograph No. 1). The highwall above the collapsed portals is <br />approximately 30 feet high with slopes inclined at approximately 0.5(11):1(V) (horizontal: <br />vertical). Rock faces above the area are somewhat steeper (Photograph No. 2). The fill slopes <br />on the edges of the pad are approximately 1.5(H):1(V). In April 2010, a collapse blocked the <br />first 15 to 20 feet of the Access Portal and the first 10 feet of the steel sets housing the mill in the <br />Mill Portal (Photographs No. 3 and 4). The portals penetrated an old landslide, which was not <br />accounted for in the original design or construction. Bedrock consists of weathered mudstone of <br />the Morrison Formation. It dips (east) into the highwall. The geologic formations that the <br />portals penetrate dip approximately 30 degrees away from the collapsed area. The collapse <br />appears be to the landslide/bedrock contact. <br />Following the collapse, MSHA issued an order ( #6445198) that prohibits all activity at the May <br />Day No. 1 Portal until MSHA has determined that it is safe to resume mining operations in the <br />area. Wildcat submitted as Exhibit D, Attachment D -2 to its amended Section 112d permit <br />application a Portal Collapse Repair Work Plan for the May Day No. 1 (the "Original Work <br />Plan"). This Original Work Plan generally addressed repair of the collapse at both the Access <br />Portal and the Mill Portal, but focused primarily on the Mill Portal. Having agreement with the <br />DRMS on the overall Work Plan, Wildcat now seeks to bifurcate its approach to rehabilitation of <br />the May Day No. 1 Portal in order to better accommodate worker safety and the concerns of <br />' In its December 10, 2009 Order addressing Notice of Violation No. MV- 2009 -035, the Mined Land Reclamation <br />Board specifically addressed a proposal by Wildcat to remove this stockpiled rock or ore from the stope accessed by <br />the May Day No. 1 Portal, providing in part that: "The Operator is ordered to cease and desist from all mining- <br />related activities, with the exception of relocating 5,000 cubic yards of ore material from the underground stope to <br />the portal pad at Level 1 ...."
