Access Portal Collapse Repair and Equipment and Stope Rock Removal Work Plan March 2, 2012 <br />May Day No. 1 <br />Quality Assurance <br />The Geotechnical Engineer and/or Technician will be onsite full time during the site work <br />activities to assure the work is performed consistent with the Regulatory Authority approved <br />construction drawings and specifications. Observations will include, but not be limited to, the <br />following: <br />• Observing cut slope excavations to confirm the slope inclinations are achieved and the <br />materials exposed are as expected; <br />• Performing the necessary laboratory testing to check that construction materials, <br />including fill and backfill materials, conform to appropriate specifications; <br />• Observing the placement of fill material; <br />• Observing construction/installation of the steel sets, and adit and highwall stability <br />features to verify they are constructed in accordance with the construction drawings and <br />specifications; and, <br />• Reviewing the contractor's daily reports and other quality control information to check <br />that work is being performed to the prescribed standards, and that unanticipated issues <br />arising during construction are promptly addressed. <br />3.3 Project Closeout at the End of Phase 2 Activities <br />The construction contractor will notify the geotechnical engineer within 5 days of substantial <br />completion. The geotechnical engineer will then schedule a final site inspection with the <br />contractor and Regulatory Authority. Work remaining and/or unsatisfactory work product <br />identified by the geotechnical engineer and/or Regulatory Authority will be discussed with the <br />contractor and memorialized in a memorandum. This memorandum will identify the `punch list' <br />items that need to be addressed for final completion. <br />At the completion of the work for Phase 2, a construction completion report will be prepared that <br />provides a brief description of the work performed and a certification that the work was <br />performed in accordance with the design and approved field changes during construction. <br />Attachments to the construction completion report will include as-built drawings, a photographic <br />log and the results of field and laboratory tests performed during construction. <br />EnviroGroup Limited <br />DEN 97,749,013v3 2 -29 -12 <br />9 <br />
