Access Portal Collapse Repair and Equipment and Stope Rock Removal Work Plan March 2, 2012 <br />May Day No. 1 <br />3.2 Quality Assurance and Control ( QA/QC) Guidelines <br />Wildcat will comply with the requirements described in the following QA/QC guidelines. Its <br />purpose is to assure that portal collapse material is removed and stabilized and adequately <br />documented. <br />Definitions <br />• Quality Control — A mechanism performed by the contractor for ensuring that <br />construction activities conform to the construction drawings and specification; <br />• Quality Assurance — A mechanism performed by the Owner to ensure that construction <br />activities conform to the construction drawings and specification; <br />• Regulatory Authority — Division of Reclamation, Mining and Safety; <br />• Geotechnical Engineer — Professional engineer licensed to practice in Colorado with <br />substantial experience in the geotechnical branch of civil engineering in general, and the <br />design and construction of adit portal stabilizations; and, <br />• Technician — An individual with training in engineering technology, with significant <br />experience in construction oversight and testing of civil and earthwork projects who is <br />trained and licensed to use nuclear density and moisture determination testing. <br />Quality Control <br />The construction contractor will generate daily reports describing work performed, equipment <br />and manpower utilized to perform the work, issues arising during the work and how these issues <br />were addressed. The contractor will be responsible for the following Quality Control activities: <br />• Providing submittals from suppliers or independent entities that demonstrate materials <br />utilized conform to the specifications; <br />• Assuring that the appropriate equipment is used to efficiently and safely achieve the lines <br />and grades shown on the construction drawings and the standards defined in the <br />specifications; <br />• Performing any necessary surveying at the site to verify dimensions, lines and grades of <br />installed and constructed features are in compliance with the design; and, <br />• Performing any necessary compaction tests or other tests specified for the design. <br />EnviroGroup Limited <br />DEN 97, 749, 013v3 2 -29 -12 <br />8 <br />
