TERMS AND CONDITIONS OF PERMIT (cont) <br />The plan identifies potential sources of pollution which may reasonably be expected to affect the quality of stormwater <br />discharges associated with industrial activity from the facility. In addition, the plan describes practices which will be used to <br />reduce the pollutants in stormwater discharges associated with industrial activiq) at the facility and to assure compliance <br />with the terms and conditions of this permit. Facilities must implement the provisions of the SWMP as a condition of their <br />permit <br />The SWMP requirements have been amended in this permit. See Section LA.3 of the Rationale for a discussion of the <br />changes. Note that some permittees will need to amend their plan to comply with the additional requirements far the <br />SWMP site map. Refer to the permit at Part LB for a complete description of the current SWAP requirements. <br />Submittal of the SWMP is not automatically required tinder this permit. However, the Division reserves the right to <br />require suhmittal upon request, and to review the SWMP and require additional measures to prevent and contra /pollution. <br />It is the Division's intent to request submission of such reports on a selective basis to determine compliance. <br />The SWMP shall include the following items, at a minimum: <br />1. Industrial activity description <br />2. Site map <br />3. Stormwater management controls, including- <br />-Name of the SWMP administrator <br />- Identification of potential pollutant sources and Best Management Practices <br />- Sampling information <br />- Preventive maintenance <br />-Good housekeeping <br />-Spill prevention and response procedures <br />- Employee training <br />- Identification of discharges other than stormwater <br />4. Comprehensive inspections <br />S. Consistency with other plans <br />B. Monitoring. Sampling and testing of stormwater for specific parameters is not required on a routine basis under this permit. <br />However, the Division reserves the right to require sampling and testing, on a case -by -case basis, in the event that there is <br />reason to suspect that compliance with the SWMP is a problem, or to measure the effectiveness of the BWS in removing <br />pollutants in the effluent. <br />C. Comprehensive Facility Inspections: The permittee will be required to make a thorough inspection of their stormwater <br />management system, at least Avice per year (in the spring and fall). Thave inspections shall be documented and summarized <br />in the Annual Report. (See Part I. C S of the permit) <br />D. Annual Report: The permittee will be required to submit an Annual Report, covering January 1 through December 31, on <br />the overall compliance with the SWMP. The Annual Report will be due to the Division on or before February 15 of the <br />following year (See Part LD. l of the permit). <br />E. Annual Fees: All permittees are required to pay the annual permit administration fee, as described in the Colorado Water <br />Quality Control Act. (See Part II.B.10 of the permit.) <br />F. Duration o Permit: The general permit may not exceed five years induration. In this case, the expiration date is set for <br />June 30, 2011. <br />The permittee's authority to discharge under this permit is approved until the expiration date of the general permit.
