If available, the Division recommends the Applicant submit a metes and bounds survey <br />description for the amendment parcels. Please revise all applicable exhibits, if required, <br />based on the response to this question. <br />Page 2 of 3 <br />which areas of the Greeley West site will not be included in the 25 Avenue Site amendment. <br />Based on the Division's review, it appears the following parcels in the Greeley West site will <br />not be included in the 25 Avenue Site amendment: <br />• Thissen II LLC Rec. No. 80536303001 <br />• Thissen II LLC Rec. No. 80536303002 <br />• Flatiron Sand and Gravel Company Rec. No. 80525000028 <br />• The Weld County 80' easement located between the Colorado Southern Railroad and <br />the southern portion of the SW Ponds area of the Greeley West Pit. <br />• Weld County Rec. No. 80536100016 — Poudre Trail easement <br />• Weld County Rec. No. 80536100017 — Poudre Trail easement <br />3. The Proposed Amendment Boundary for the 25 Avenue Site differs from the approved <br />permit boundary for the Greeley West site. Please confirm the areas indicated in Question #2 <br />above will remain in the Greeley West permit. Additionally, please confirm the "notch" area <br />located adjacent to 25 Ave which was previously not included in the Greeley West site will <br />be included in the 25 Avenue Site through this amendment. A copy of the 2011 Annual <br />Report map for the Greeley West site is attached for reference. Please note the map is <br />incorrectly labeled "2009 Annual Report Map" <br />4. The contact name listed on page 3 of the application form, Jim Hall, differs from the name of <br />the signer of the application, Jon Monson. The Applicant must provide documentation giving <br />the signer the authority to sign on behalf of the City of Greeley and explain this discrepancy. <br />The authorization document may be in the form of a notarized statement on City of Greeley <br />letterhead. <br />5. The Applicant /Operator name on the Certification page must match exactly with the name <br />listed on Item #1 of the application form and with the current Permittee listed with the <br />Division for the site. Please revise the "Applicant /Operator or Company Name" section on the <br />certification page (page 8) to indicate "City of Greeley" as the Applicant. <br />6.4.1 Exhibit A — Legal Description <br />6. Please revise the acreages listed in Exhibit A based on the Applicant's responses to Questions <br />#1 and #2 above. <br />6.4.2 Exhibit B — Index Map <br />7. Please revise the proposed amendment site on Exhibit B based on the Applicant's response to <br />Question #3 above and indicated the current 25 Avenue Site permit boundary on the map. <br />2 <br />
