The Golden Eagle site consists of a <br />reclaimed Ventilation fan and surrounding pad area, and reclaimed access roads. The rest of the Golden Eagle <br />operation has previously received final bond release. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />Golden Eagle Mine records are stored at the New Elk Mine office in Trinidad. Records were inspected. A <br />Rill and Gully Report is still needed to be filed for 2011. Records checklist is included with this inspection report. <br />HYDROLOGIC BALANCE -Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, <br />4.05.10, Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br />4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The Golden Eagle NW vent shaft site consists of a graded pad around the sealed and regraded ventilation <br />shaft. The site is graded gently eastward, is designed and functions as an SAE. The site is well vegetated with <br />native grasses, and shows no evidence of rills or gullies. An upland diversion remains in place upslope from the <br />reclaimed site. Drainage from the logging operation to the north is directed onto the Golden Eagle site. This ditch <br />intercepts the Golden Eagle NW vent fan site east (downslope) of the Golden Eagle disturbance area limits. No <br />rills were apparent from this additional drainage being routed onto the Golden Eagle site. <br />No problems were identified with the upland diversion ditch to the west of the reclaimed vent shaft pad. <br />GENERAL MINE PLAN COMPLANCE: <br />No problems were observed on the reclaimed area during the inspection. <br />Number of Partial Inspection this Fiscal Year: 5 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 2 of 6 <br />
