Please be advised that if you are unable to satisfactorily <br />address any concerns identified in this review before the decision date, it will be your <br />responsibility to request an extension of the review period. If there are outstanding issues <br />that have not been adequately addressed prior to the end of the review period, and no <br />extension has been requested, the Division will deny this application. <br />1. The Applicant has not responded to the Division's Preliminary Adequacy Review letter sent <br />on February 17, 2011. As stated above, it is your responsibility to address all of the <br />Division's adequacy concerns prior to the decision date. Please respond to the Division's <br />adequacy concerns provided in the Preliminary Adequacy Review letter (copy enclosed) as <br />well as the additional items listed below. <br />6.3.5 Exhibit E — Map <br />STATE OF COLORADO <br />John W Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Loretta E Pineda <br />Director <br />2. The Division received the Permit Map submitted on December 7, 0211. Please revise the title <br />of this map to `Mining Plan Map'. <br />3. The permit boundary must be labeled on the Mining Plan Map, please revise the map <br />accordingly. <br />4. The Mining Plan Map states the total permit area is 10 acres. Please note that under a 110 <br />Reclamation Permit, the maximum area which can be permitted must be less than 10 acres. <br />The Applicant has indicated on the Application Form that the revised permit acreage is 9.95 <br />acres.
