<br />With respect to mail and other coromimiattions: To open, read, respond
<br />to and redirect my mail; to represent me before the U.S. Postal Service in

<br />(7) With respect to memberships: to establish, cancel, continue orinitiate my
<br />membership in organizations and associations ofall kinds,
<br />(8) . With respect to document and records: to take and give or deny custody
<br />of all of my important documents, including but not limited to my will,
<br />codicils, trust agreements, deeds, leases, life insurance policies, contracts
<br />and securities and to disclose or refuse to disclose such documents; to
<br />obtain and release or deny information or records ofall kinds relating to me,
<br />any interest of mine or to any person for whom I am responsible; to
<br />authorize the release of the information requested by my agent under the
<br />Freedom of Information Act, the Open Records Act and the Health
<br />Insurance Portability and Accountability Act of 1996 (a.k.a. HIPAA), 42
<br />U.S_C.1320(d) and 45 C.F.R. 160 -164 and under any other law and waive
<br />all rights ofprivacy and confidentiality. This authority shall remainin effect
<br />for so long as this durable power of attorney remains in effect and has not
<br />been revoked.
<br />With respect to animals: to house or provide for housing, support and
<br />maintenance of any animals or other living creatures that I may own and to
<br />contract for and pay the expenses of their proper veterinary care and
<br />treatment; and if the care and maintenance of such animals or other living
<br />creatures shall become unreasonably expensive or burdensome in my
<br />Agent's opinion, to irrevocably transfer such animals to some person or
<br />persons willing to care for and maintain them.
<br />(10) With respect to health cadre power of attorney: My agent is expressly
<br />authorized to provide adequate funds for payment of my health care, social,
<br />and companionship needs.
<br />(11) Wi1Ii respect to Oil, Gas and Mineral Interests: To sell, convey, lease,
<br />contract, and otherwise dealt with any oil, gas or other mineral interests of
<br />any kind whatsoever, and wherever situated, which I may own, or which I
<br />may acquire in the future. This power shall include, but shall not be limited
<br />to, the power to execute oil, gas, or other mineral or royalty deeds, leases,
<br />transfer and division orders, joint or operating agreements, utilization
<br />agreements, gas sales contracts or other future oil and gas business
<br />transactions.
<br />(12) With respect to Gifts: To make gifts of anyof my assets to any individuals
