10. The Applicant has submitted a map labeled as `Exhibit C'. Please note that Exhibit C pertains <br />to the Mining Plan. This map may be utilized as the Mining Plan Map; however, the <br />following revisions will need to be made: <br />a) The map must contain a scale. The acceptable range of map scales shall not be larger <br />than 1 inch = 50 feet nor smaller than 1 inch = 600 feet. <br />b) The map must contain a title (e.g., Mining Plan Map). <br />c) The map must be prepared and signed by a registered land surveyor, professional <br />engineer, or other qualified person. <br />d) The map must give the date it was prepared. <br />e) The map must show the name of the Applicant. <br />f) The map must contain labels for the names of owner(s) of record of the surface of the <br />affected area and of the land within 200 feet of the affected area and for the type of <br />structure and owners of record of any permanent man-made structures within 200 feet <br />of the affected area. <br />g) The map must outline and label the pit boundary, topsoil stockpiles, product <br />stockpiles, overburden stockpiles and stream channels. <br />h) The map must indicate the direction material extraction will proceed. <br />Mr. Chartier <br />December 12, 2011 <br />Page 13 <br />7. Please specify the areas which will need to be seeded upon final reclamation and state the <br />total area requiring seeding. <br />8. As required by Rule 6.3.4(1)(c)(ii), please explain how the seedbed will be prepared to <br />eliminate compacted conditions prior to seeding (e.g., plowed, chiseled, disced). <br />9. As required by Rule 6.3.4(1)(c)(v), please specify the kind of mulch to be used, the crimping <br />method, and rate of application. <br />6.3.5 Exhibit E — Map <br />11. The Applicant did not submit a Reclamation Plan Map. The Reclamation Plan map must <br />contain the following elements: <br />a) The map must contain a scale. The acceptable range of map scales shall not be larger than <br />1 inch = 50 feet nor smaller than 1 inch = 600 feet. <br />b) The map must contain a title (e.g., Reclamation Plan Map). <br />c) The map must be prepared and signed by a registered land surveyor, professional <br />engineer, or other qualified person. <br />d) The map must give the date it was prepared. <br />e) The map must show the name of the Applicant. <br />f) The map must show the gradient of all reclaimed slopes (horizontal:vertical) sufficient to <br />describe the post mine topography. <br />g) The map must indicate where vegetation will not be established. <br />h) The map must show where the berm will be located. <br />
