CQA ACTIVITIES: <br />SUMMARY: <br />200 excavator and a John Deere 544 front -end loader with spreader -bar attachments. Subgrade <br />was inspected and approved by AMEC and Lonestar representatives prior to geomembrane <br />deployment (see Figure 3). Approximately 18,444 linear feet of seam was fusion welded using <br />five fusion welding machine /welding technician combinations. Forty one destructive test <br />samples were marked during fusion seaming. Approximately 1,368 linear feet of seam was <br />extrusion welded using two extrusion welding machine /welding technician combinations. Five <br />destructive test samples were marked during extrusion seaming. All destructive tests performed <br />were within project specifications. Non - destructive testing was performed for fusion welded <br />seams. Repairs were performed using the extrusion welding method for all marked defects. <br />Vacuum testing was performed for all extrusion welded seams and repairs. <br />During the Cresson blasts, an AMEC field representative observed the geomembrane and no <br />damage was noted. <br />B) Geomembrane Exposure <br />1) VLF <br />No work performed during the week. <br />C) Anchor Trench <br />1) VLF <br />Anchor trench excavation continued between stations A16 +00 ( + / -) and A31 +00 ( + / -), near <br />centerline, using a John Deere 35D excavator and one laborer. The anchor trench was <br />inspected by an AMEC field representative and found to be constructed in accordance with <br />design drawings and project specifications. <br />Anchor trench backfill was performed between stations A8 +50 ( + / -) and A22 +00 ( + / -), near <br />centerline, using a John Deere 35D excavator, a vibratory plate compactor and laborers. An <br />AMEC field representative observed the backfill and it was found to be constructed in <br />accordance with design drawings and project specifications. <br />D) Geomembrane Acceptance <br />1) VLF <br />Primary 80mil geomembrane (P1172- P1212) shown on Figure 4 was inspected by AMEC and <br />Lonestar representatives and found to be installed within project specifications and acceptable <br />for continued construction activities. <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed snow removal within the Phase 5 VLF. <br />AMEC observed and assisted in the placement of SLF material within the Phase 5 VLF. <br />VLF Weekly Report WE 10.22.2011 <br />
