Mr. Striggow <br />Page I2 <br />November 2, 2011 <br />6.4.3 Exhibit C — Pre- mining and Mining Plan Map(s) of Affected Lands <br />3. The Operator has indicated this Exhibit remains unchanged from the original application. <br />The original Mining Plan map does not depict the amended area. Please submit a revised <br />Mining Plan Map which portrays the following: <br />a) All adjoining surface owners of record. <br />b) The name and location of all creeks, roads, buildings, oil and gas wells and lines, and <br />power communication lines on the area of affected land and within 200 feet of all <br />boundaries of such area. <br />c) The existing topography of the area with contour lines of sufficient detail to portray the <br />direction and rate of slope of the affected land. <br />d) The total area to be involved in the operation, including the area to be mined and the area <br />of affected lands. <br />e) The type of present vegetation covering the affected lands. <br />f) Show the owner's name, type of structures, and locations of all valuable, and permanent <br />man-made structures contained on the area of affected land and within 200 feet of the <br />affected land. <br />g) Show the name of the Applicant. <br />h) Be prepared and signed by a registered land surveyor, professional engineer or other <br />qualified person. <br />i) Give date prepared. <br />j) The acceptable range of map scales shall not be larger than 1 inch = 50 feet nor smaller <br />than 1 inch = 600 feet. Also, include an appropriate legend, map title, and a north arrow. <br />6.4.4 Exhibit D — Mining Plan <br />4. Under the approved Reclamation Permit the Operator can have a maximum disturbed area of <br />38.75 acres. Please clarify if the Operator will maintain a maximum disturbed area of 38.75 <br />acres or if the maximum disturbed acreage will increase as a result of this amendment. <br />5. Please clarify where the overburden from the amended area will be stockpiled. Will the <br />overburden be placed in the stockpile on the east side of the permit area or in another <br />location? <br />6. As required by Rule 6.4.4(e), please provide a revised timetable to describe the mining <br />operation. The timetable is for the purpose of establishing the relationship between mining <br />and reclamation during the different phases of a mining operation. <br />7. Please specify the depth of deposit to be mined for the amended area. <br />6.4.5 Exhibit E — Reclamation Plan <br />8. The Operator has stated that topsoil will be salvaged and placed on the west edge of the <br />amended area. Since the material will be placed along a ridge, the Division requests the <br />
