Function of Amendment: Structure of Amendment: Amendment Structure and Overview Purpose of Amendment: To add 6,762 +/- 200 square feet (0.16 acres) to the affected land area. There is no change in the permit area. 1. To include a small parcel of land that was affected beyond the affected land boundary identified in the original permit. 2. To provide information on the corrective actions taken to mitigate the disturbances made beyond the original permitted affected land boundary. The removal of spilled rock and reconstruction of damaged land implemented as a part of the corrective actions were completed on August 4, 2011. 3. It is important to note that this amendment DOES NOT add any land to the permit for this operation. The disturbances created beyond the affected land boundary were wholly contained within the permitted land. This amendment includes the necessary exhibits, as listed in the application form and described in the rules and regulations, for the amendment area only and any immediately adjacent permitted land where the corrective actions that have been applied need modification as a result of the corrective actions implemented. No other portions of the permit are modified as a result of this amendment. Many of the original exhibits provided in the permit application still apply and do not require modification as a result of this amendment.
