Mr. Mike Boulay <br />Sincerely, <br />October 21, 2011 <br />SCC: I spoke to Susan Burgmaier on this issue and the AVS system has been updated <br />verifying the correct status of the officers /directors as per Snowcap's Appendix 3 -1. <br />However, there has been one officer termination (Susan Tomasky — 07/31/11) for <br />American Electric Power Company, Inc. since the original technical revision application <br />was submitted on July 18, 2011, so revised Appendix pages A3 -1 -1 through A3 -1 -5 are <br />enclosed. These pages were also revised showing corrected phone numbers and CO- <br />AVS-ID numbers. <br />3. DRMS: With SL -7, the Division approved the removal of Unit Train Loadout and Rollins <br />Sandstone groundwater monitoring wells. These were properly plugged and abandoned <br />and reports submitted to the Division. Exhibit 56 was revised with TR -62. The reclaimed <br />wells are still shown on this map which is fine. However, the Division requests that the <br />nomenclature in the legend be changed from "inactive" to "reclaimed" for all wells that <br />have been plugged and abandoned. <br />SCC: Exhibit 56 is enclosed and has been revised changing the legend to include <br />"reclaimed" wells. <br />4. No response necessary. <br />5. No response necessary. <br />6. DRMS: The North Decline Post Mining Topography Map (Exhibit 12C) was updated with <br />TR -62. Based on our review and recent site visit we are requesting the following <br />additional change to the map and revision to permit text. The 18" V -Ditch located east of <br />the main permanent channel should be removed from the map since this is scheduled to <br />be reclaimed along with Pond 7. The two 12" V- Ditches that flow to the permanent <br />channel and the large berm at the east perimeter should remain on the map because <br />they are permanent features.
