d. Figure D -1, Environmental Protection Plan, delineated a permit boundary
and boundary of affected lands which is different from all other maps of
the application. Figure D -1 also delineated affected lands located outside
of the permit boundary and on lands owned by Doyle E. Huckabay and
the United States of America. The application did not demonstrate a legal
right to enter lands owned by Doyle E. Huckabay or the United States of
America. Pursuant to Rule 6.4.14, the application is required to describe
the source of the Applicant's legal right to enter and initiate a mining
operation on the affected lands. If Figure D -1 is correct, please revise all
other maps of the application to reflect a consistent boundary and
describe the source of the Applicant's legal right to enter. If Figure D -1 is
incorrect, please submit a correct Figure D -1.

Response to Adequacy Issue No. 4.d.
Wildcat is submitting a revised copy of Figure D -1, Environmental Protection
Plan, correcting the permit boundary which inadvertently appeared differently than on
other maps. See Ex. 1. Wildcat hereby withdraws all other versions of Figure D -1,
Environmental Protection Plan, including those submitted on September 9, September 27
and October 18, 2011.
Supplemental Response to Sixth Adequacy
Adequacy Issue No. 1.
Pursuant to Rule 1.4.1(3), all information submitted in an application must be
accurate and complete, and acknowledged as such by the signature of an authorized agent
on the application form.
The application form received on September 9, 2011, was not properly completed
by the Notary. The Notary failed to identify the individual who signed the application.
The Division received a correct application form with the amendment to CN -01,
filed April 27, 2011. However, the previously accepted form was replaced on August 9,
2011, and again on September 9, 2011. Please re- submit the form previously accepted
with the amendment, filed April 27, 2011, or submit a new form properly executed in
accordance with the requirements of Rule 1.4.1(3).
