Engineering Evaluation <br />For CenturyLink, Inc., f/k/a, Qwest Communications, Inc. <br />Telephone Utility Box <br />By Daniel Madruga, P.E. <br />CLC Associates, Inc. <br />Description of Man Made Structure: <br />CenturyLink, Inc., f/k/a Qwest Communications, Inc., owns a telephone utility box which <br />is located adjacent to County Road 124, approximately 30 feet north of the New Access Road, <br />located approximately between Line Stations 10 +00 and 11 +00. <br />Engineering Evaluation: <br />I present the following engineering evaluation to mitigate or avoid any potential damage <br />to the CenturyLink, Inc. telephone utility box. I have designed the New Access Road between <br />Line Stations 10 +00 and 11 +00 consistent with the Clear Zone concept specified in the <br />American Association of State Highway and Transportation Officials (AASHTO) - A Policy on <br />Geometric Design of Highway and Streets. In this Policy, the term "Clear Zone" is used to <br />designate the unobstructed, relatively flat area (less than 10% grade) provided beyond the edge <br />of the traveled way for the recovery of errant vehicles. The clear zone includes any shoulders <br />or auxiliary lanes. For low -speed rural collectors or rural local roads, a minimum clear zone <br />width of 10 feet should be provided. The distance between the New Access Road and the <br />CenturyLink, Inc. telephone utility box is approximately 30 feet, and the grade along this <br />distance is approximately four percent, sloping from the New Access Road to the CenturyLink, <br />Inc. telephone utility box. In my professional opinion, Wildcat's design, which meets this <br />Policy, will create an appropriate buffer zone which will prevent any substantial risk of damage <br />to the CenturyLink, Inc. telephone utility box as a result of activities occurring at the Wildcat <br />mining operation. <br />Certified BY: <br />DEN 97,636,867v1 10 -19 -11 <br />
