Provide a descriptive report interpreting the results of geophysical logs and other tests. Include a <br />description and data on deviation checks run during drilling. <br />Please see following AMSO reports submitted to Wendy Cheung (EPA), Travis Marshall (DRMS), and Kent <br />Walter (BLM) for the required logging and testing requirements specified for the production /injection <br />well T -1P in the UIC Permit: <br />• July 26, 2011— Casing cement operations /evaluations and gamma ray log <br />• August 15, 2011 — Step rate test and resistivity and porosity logs <br />• October 6, 2011— Standard annulus pressure test, baseline temperature log (fiber optic cable in <br />conduit cemented outside 9 5/8" casing), and fiber optic cable conduit pressure test (conduit <br />cemented outside 9 5/8" casing). <br />Deviation checks during drilling are described in Exhibit A (Keeper Gyro Survey Report prepared by <br />Scientific Drilling for Well T -1P. <br />VI. Provide an as -built diagrammatic sketch of the injection well(s) showing casing, cement, tubing, <br />packer, etc., with proper setting depths. The sketch should include well head and gauges. <br />Please see attached Figure VI -1 (T -1P Well Completion Diagram). <br />VII. Provide data demonstrating mechanical integrity pursuant to 40 CFR 145.08. <br />Please AMSO's October 6, 2011 report submitted to Wendy Cheung (EPA), Travis Marshall (DRMS), and <br />Kent Walter (BLM) regarding the standard annulus pressure test conducted at production /injection well <br />T -1P. <br />VIII. Report on the compatibility of injected wastes with fluids and minerals in both the injection zone <br />and the confining zone. The AMSO test is not injecting wastes into the injection zone. <br />IX. Report the status of corrective action on defective wells in the area of review. <br />None of the area of review wells is defective. <br />X. Include the anticipated maximum pressure and flow rate at which injection will operate. <br />Maximum pressure: 1,025 psi <br />Maximum flow rate: No permit limit established because naturally produced products will be <br />recirculating. <br />
