(11) Section 8, Surface water, indicates an unnamed perennial stream 1,270 feet from the mine. <br />The head of this stream indicates the possibility of a spring from groundwater at an elevation <br />below the mine workings. An onsite visit notes that there is a drainage there, but no indications <br />of a perennial stream. Cotter needs to clarify the map and actual site conditions through a <br />drainage reconnaissance report. The Division may require five quarters of sampling under Rule <br />6.4.21(11) (b) if a spring or seep is discovered as indicated on the maps. <br />(12)Section 10, the Construction Schedule and the associated Table 24 have an error that effects the <br />entire information presented. As noted in the mine permit review, the status of the production <br />of the mine has no bearing on the EPP requirements. Site characterization and stormwater <br />control structures shall be required to be installed regardless of any other activities on site upon <br />approval of the EPP plan. Cotter must submit a construction schedule that identifies when these <br />activities will be initiated upon approval of the EPP not "contingent upon resumption of active <br />mining ". <br />(13)The EPP does not address Quality Assurance and Quality Control as required under Rule <br />6.4.21(16) and Rule 7.3.1(4). <br />(14)Rule 7.3.2, written acceptance of certification for facilities under this paragraph (Environmental <br />Protection Facilities) must be provided by the operator via a certified professional engineer or <br />other appropriate qualified professional. None of the proposed facilities has an engineer's <br />stamp or certification of design. <br />(15)Table 26, the seed mix and associated text should also be noted in the Reclamation Plan, <br />Exhibit E. <br />(16)A map of sufficient size, that clearly shows stormwater control structures, is needed. The map <br />should show stormwater run -on prevention structures, a stormwater collection system within <br />the permit area and affected areas, and their relationship to containment structures. The map <br />with limited arrows is too general. <br />(17)Please provide a detailed map of the ore storage pad area that highlights stormwater retention, <br />and run -on, run -off structures. Please detail measures that will be put into place to prevent <br />cumulative collection and infiltration of radioactive sediments into the substrata below the <br />footprint of the stockpile area. A geo- synthetic liner or some other approved barrier for the <br />area needs to be part of the EPP. <br />(18)AII EPP maps appear to show growth media stockpiles and waste rock piles overlapping. Please <br />correct. <br />
