Box 68 <br />Empire, CO 80438 <br />Re: Climax Molybdenum Company, Henderson Mine, Permit No. M- 1977 -342 <br />Technical Revision No. TR -09 Approval <br />Dear Mr. Hamarat: <br />As requested by the Division of Reclamation, Mining and Safety (Division /DRMS) during the August <br />23, 2011 meeting at the Henderson Mill, Climax Molybdenum Company (CMC) provided an <br />additional copy of the following documents related to Technical Revision No. 9 (TR -09) of the <br />Henderson Reclamation Permit. <br />Mined Land Reclamation <br />• 2 -Dam Grouting and 3 -Dam Seep Water Management <br />STATE OF COLORADO <br />Stipulation No. 1–Technical Revision 9 <br />The 3 -Dam seepage pipelines will not be put into service until the Division has issued <br />written acceptance of the certification of the facility as required under rule 7.3.2(2) <br />of the Hard Rock /Metal Mining Rules and Regulations. The certification shall be <br />accompanied by a construction report that will detail the installation of the pipeline, <br />including pipe and valve specifications, a description of the inlet structures, seepage <br />barriers and south dike reconstruction, a description of any problems that occurred <br />and their resolution, and a description and test results of pipeline integrity testing. <br />Stipulation No. 3 –Technical Revision 9 <br />Within 60 days of completion, a grouting report will be provided for the 2 -Dam <br />decant system grouting operation. The report will detail the installation of the <br />grout, include a description of any problems that occurred and their resolution, and <br />will include compressive strength test results from the cured grout samples. <br />COLORADO <br />D I V I S I O N O F <br />RECLAMATION <br />MINING <br />—&— <br />SAFETY <br />John W.
