May Day /Idaho Mine — Comments to Selected Work Plans from the August 2011 Submittal, <br />Exhibit D <br />Page 2 <br />August 22, 2011 <br />schedule is tied to the date site access might be granted and seasonal limitations related to <br />field and construction work. <br />3. Figure D -2, Stormwater Control Plan: <br />a. The Type "C" inlet labeled with a hexagon enclosing a "1" appears to be capable <br />of intercepting very little upgradient stormwater runon. Please provide some <br />discussion as to how runon is to be intercepted or diverted away from the north <br />side of the May Day No. 1 pad. <br />b. The south diversion ditch in the plan view flowing north towards the sediment <br />basin appears to be flowing uphill, gaining five feet in elevation. Please provide <br />some explanation or revise the grading plan. <br />Attachment D -3, Little Deadwood Gulch May Day No. 2 Drainage Control <br />4. Page 2, Management. The second bullet attempts to ensure DRMS direct supervision <br />will be provided. This statement shall be revised to something similar to that stated in <br />Attachment D -4, "Coordinate schedules with DRMS, and offer its representatives the <br />opportunity to inspect construction work activities ". <br />5. Page 3, Construction. Riprap specifications pertaining to gradation, density, and shape <br />shall be provided. <br />6. Page 3, Construction. No specifics are provided for culvert installation. Culverts are to <br />be installed on proper bedding (reference CDOT specifications for example) and shall be <br />placed no closer than '/ diameter apart. <br />7. Page 5, Quality Assurance. The following items do not appear to be relevant to the work <br />plan and should be removed, or provide some narrative describing how they are relevant: <br />a. Second bullet related to retaining wall foundation excavations and roadway over <br />excavation. <br />b. Third bullet related to laboratory testing. No borings /test pits'are proposed in the <br />work plan, so there is apparently nothing to test. <br />c. Fourth bullet related to fill material placement in 8 -12 inch lifts and field density <br />tests. This should be removed unless this is the planned approach for the 18 <br />inches of cover over the culverts. <br />8. Page 5, Quality Assurance. There is no discussion on ensuring culverts are installed to <br />grade on appropriate bedding or that riprap is placed mechanically or by hand to prevent <br />segregation. <br />9. Page 6, Reporting. The fifth solid bullet indicates "as- built" drawings will be prepared in <br />the third quarter of 2011. DRMS recommends this statement be revised such that the <br />schedule is tied to the date site access might be granted and seasonal limitations related to <br />field and construction work. <br />10. Figure D -1, Stormwater Control Plan: <br />a.
