RE: Fifth Adequacy Letter for a 112d -1 Application, Amended CN -01,
Wildcat Mining Corporation, May Day Idaho Mine Complex, Permit No. M- 1981 -185.
Dear Mr. Robinson:
On August 9, 2011, the Division received response from the Applicant to the Division's previous
four adequacy letters and to the issues discussed during a meeting occurring on June 29, 2011.
Please be advised the amended CN -01 may be deemed inadequate and may be denied on
September 26, 2011, unless the adequacy review items listed below, and those identified in the
four attached technical review memorandums from staff members David Bird, Peter Hays and
Tim Cazier, are addressed to the satisfaction of the Division. Many of these adequacy issues
have been expressed multiple times, in previous adequacy letters and discussed at length in the
full day Wildcat/ DRMS meeting held on June 29, 2011. Subsequent to receipt and review of
the Applicant's response to these items the Division may identify additional issues.

Adequacy Issues for the August 9, 2011' Submittal
1. Pursuant to Rule 1.4.1(3), all information submitted in an application must be accurate
and complete, and acknowledged as such by the signature of an authorized agent on the
application form.
The application form received on August 9, 2011, contained errors in permit acres,
provided incorrect contact information for the Applicant, and was not signed by an
authorized agent. The Division had previously reviewed this same application form,
received on September 29, 2010, and had rejected the form due to the errors.

The Division received a correct application form with the amendment to CN -01, filed
April 27, 2011. However, the previously accepted form was replaced on August 9, 2011.
Please re- submit the form previously accepted with the amendment, filed April 27,
2011, or submit a new form properly executed in accordance with the requirements of
Rule 1.4.1(3).
