H. The CQA Monitor shall prepare weekly progress reports summarizing all <br />construction and CQA activities. This report shall be submitted to the <br />Project Manager and shall include the following information: <br />• Date, project name, and location; <br />• Summary of construction related activities; <br />• Summary of geosynthetic deployed (per day); <br />• Summary of samples taken and test results; <br />• Summary of geosynthetic areas completed, and approved <br />for liner protection fill placement; <br />• Summary of deficiencies and /or defects and resolutions; <br />and <br />• Signature of the CQA Monitor. <br />At the completion of the project, the CQA Monitor shall submit to the <br />Project Manager a Construction Certification Report. This report shall <br />certify that the work has been performed in compliance with the design <br />Drawings and project Specifications and will contain the following <br />information: <br />• Summary of all construction activities; <br />• Observation and test data sheets; <br />• Photographic documentation; <br />• Contractor's subgrade acceptance forms; <br />Temperature monitoring results; <br />Geosynthetic QC documents; <br />Geosynthetic QA documents; <br />• Geosynthetic installation observations, such as for <br />deployment, trial seams, defect repair, destructive testing <br />and non - destructive testing; <br />• Sampling, testing locations, and test results; <br />• Changes to the design Drawings or project Specifications <br />and the justification for these changes; <br />• Record Drawings; and <br />• A certification statement that construction was completed <br />in compliance with the Drawings and Specifications, <br />signed, and sealed by the Design Engineer. <br />Rio Grande Silver Tailings Evaporation Pond 01450 -12 Quality Assurance /Quality Control <br />August2011 01450 <br />
