Compliance With Mine Plan <br />R - Other <br />NA - Processing Waste <br />R -Roads <br />Y - Reclamation Success <br />R - Revegetation <br />NA - Subsidence <br />Y - Slides and Other Damage <br />NA - Support Facilities On -site <br />Y -Signs and Markers <br />NA - Support Facilities Not On -site <br />NA - Special Categories Of Mining <br />Y - Topsoil <br />COMMENTS <br />This was a partial inspection of the Hamilton Mine conducted by Marcia Talvitie of the DRMS. <br />The inspection commenced at 8:20 a.m. on Thursday, July 28, 2011, and concluded at 9:10 a.m <br />Skies were clear, and the temperature was warm. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures <br />4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; <br />Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments <br />4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />• Ground conditions were damp throughout the general area, including the permit acreage. <br />The local newspaper, the San Miguel Basin Forum, has reported the occurrence of significant <br />recent thunderstorm events in the nearby towns of Naturita and Nucla. <br />• All slopes and ditches appear to be stable, and no signs of erosion were observed. <br />• Standing water was observed in Sedimentation Pond B for the first time this year. The <br />northern end of the pond was covered with approximately 2 inches of water. <br />• Inlet and outlet ends of culvert pipes were clear of sediment and debris. <br />ROADS — Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and Maintenance <br />4.03.1(5) and (6)/4.03.2(5) and (6); Reclamation 4.03.1(7)/4.03.2(7): <br />• Wheel paths of the haul road leading to Mining Area No. 3 were muddy with ponded water <br />in the sag vertical curve where the roadway crosses the natural drainage upstream of Pond B <br />(Photo). This condition effectively prohibited vehicular access to the eastern extent of the <br />permit area. <br />• Plan and profile for this haul road are presented on Permit Drawing No. 131. The low point <br />in the sag vertical curve occurs at approximately Sta. 17 +00. According to the Typical <br />Section on the drawing, the road is to be surfaced with four to six inches of gravel. This haul <br />road is specifically addressed in the permit text on pages 2.05 -Iii and 7iii, and the landowner <br />Number of Partial Inspection this Fiscal Year: 1 <br />Number of Complete Inspections this Fiscal Year: 0 Page 2 of 6 <br />
