Emergency Response and Management Plan <br />Rule 8 <br />7) In accordance with the Mine Notification Plan, immediately notify San Juan Mine <br />Rescue Coop, in event all personnel cannot be accounted for to implement Mine <br />Rescue operations. <br />6. Emergency Response to Fire /Avalanche /Chemicals: <br />In the event of a fire, dry chemical fire extinguishers will be used as the first measure of <br />containment, and the local fire district will be notified immediately. Should the need arise for <br />emergency vehicles onsite; a ground guide will be dispatched to meet said vehicles at County <br />Road 124. Access to the site will be as shown on the Emergency Plan (Exhibit C, Figure C -5). <br />In the event of a forest fire or avalanche, evacuations will occur using the safest route. Fort <br />Lewis Mesa Fire Protection District has established a landing zone for emergency helicopter <br />rescue which is located adjacent to the May Day Idaho Mine Complex base of operations. <br />In the event of a chemical spill or accident the following will be conducted: <br />• Notify Colorado Department of Public Health and the Environment, the National <br />Response Center, the Colorado Emergency Planning Commission and the La Plata <br />County emergency response agency of the accident including details related to: <br />o Operation name, DRMS permit number and the name of person reporting the <br />spill; <br />o Location; <br />o Time; <br />o Date; <br />o Responder; <br />o Telephone number of the material spilled (CAS number from applicable MSDS <br />forms); <br />o Nature of Emergency Condition; <br />o Chemicals of Concern; <br />o Quantity of Chemicals involved; <br />o Description of potential impacts to human health and the environment; and <br />o Initial measures taken to contain and clean up spill. <br />• Reports will be within 24 hours of the time the spill occurred to DRMS and the County. <br />• Prepare a formal report which: <br />o Provides spill /accident details; <br />o Name, address and telephone number of the responder(s); <br />o Known or anticipated adverse human health or environmental impacts; <br />o Activities taken to monitor or mitigate the conditions; and <br />o Corrective action to mitigate current and future similar spills or accidents. <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August 2011 <br />Rule 8 <br />Page 8 of 14 <br />
