SOP No. 5.0 Monitoring Well Installation <br />6.2 MONITORING WELL COMPLETION INFORMATION <br />The well completion information will be recorded on the Screening and Water -level forms or <br />field log forms. The forms will include the following information: <br />• Well identification (identical to the borehole identification) <br />• Drilling method <br />• Installation date(s) <br />• Total boring depth and total well depth <br />• Lengths and descriptions of the screen and casing <br />• Depths and descriptions of the filter pack, bentonite seal and casing grout <br />• Elevation of water surface before and immediately after well installation <br />• Summar) of the material penetrated by the boring <br />• GPS reading using correct coordinate system <br />7.0 REFERENCES <br />U.S. EPA. 1986.
