New Access Road Construction Work Plan <br />Exhibit D — Attachment 1 <br />Temporary Sediment Traps /Rock Check Dams <br />• Where necessary, check dams or debris basins will be constructed to minimize <br />surface water erosion to impacted areas (See Attached Guidance Documents for <br />installation guidelines) (Attachment D -1 - Sheet C4.92 for Best Management Practice <br />illustration). • <br />Road Base <br />• Final grading will include a suitable crushed rock road base ( -3 inch) on the access <br />road placed over a crushed eight inch thick road base (See Attachment D -lat Sheet <br />C2.00). <br />Berms /Guard Rails <br />• A berm/guard rails will be constructed on the outside of the road fill; <br />• The berm/guard rails will be at least 1.5 to 2 feet high; <br />• The berm/guard rails will be constructed in accordance with Mining Safety and <br />Health Administration (MSHA) and CDOT guidelines; and <br />• Guard rails will be constructed in accordance with Standard Engineering Practices <br />from Station 18.10 to 19 +90 (See Attachment D -1 at Figure C- 4.12). <br />Reclamation Specifications <br />• Reclamation on terrace deposits cut and fill slopes will be graded, revegetated <br />(including trees). (See Seed Mix /Planting program as presented in Exhibit E.); and <br />• As appropriate, cut slopes on unconsolidated materials steeper than 1(H):1 (V) will be <br />reinforced (Attachment D -1 at Sheet C4.91). <br />Contingency <br />• If the road slumps or partially fails, the failed area will be repaired by backfilling the <br />area with suitable material. If the materials impacts the wetland at the base of the fill, <br />Wildcat will mitigate the impacted area by creating wetland offsets similar to the <br />offsets currently required by the Army Corp of Engineers (Application number SPK- <br />2007- 980 -CD) where the offsets have been at establishing a 1 to 1 (Impact to <br />Mitigation) area. <br />Quality Control and Assurance (QA/QC) Guidelines <br />• Wildcat will comply with the requirements described in the following QA/QC Plan. <br />Its purpose is to assure that access road construction and reclamation is implemented <br />in accordance with the DRMS and County- approved construction drawings and <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August, 2011 <br />Exhibit D — Attachment 1 <br />Page 10 of 15 <br />
