The bond is rated as poor from the lower Uinta Formation 396 ft bgs to the top <br />of the logged interval (26 ft bgs). <br />TM -2 (Attachment C) <br />CAST -M log completed: May 5, 2011 <br />Casing Depth: 2,290 ft bgs <br />Interval Logged: 46 — 2,243 ft bgs <br />The ACE report prepared by Halliburton indicates the cement to pipe bond is adequate from the bottom of <br />the log (2,243 ft bgs; upper RO) to the middle of the Uinta Formation (310 ft bgs). The bond is rated as <br />fair from the bottom of the log (upper RO) to lower Uinta Formation (358 ft bgs), good through the <br />middle of the Uinta Formation (358 — 310 ft bgs), and poor within the upper Uinta Formation (310 ft bgs <br />to the top of the logged interval [46 ft bgs]). <br />TM -3 (Attachment D) <br />CAST -M log completed: May 5, 2011 <br />Casing Depth: 2,215 ft bgs <br />Interval Logged: 46 — 2,098 ft bgs <br />The ACE report prepared by Halliburton indicates the cement to pipe bond is adequate from the bottom of <br />the log (2,098 ft bgs; lower R1) to near the top of the R8 (566 ft bgs). The bond is rated as good from the <br />bottom of the log (lower R1) to near the top of the R8 zone (566 ft bgs) and poor to adequate from the top <br />of the R8 (566 ft bgs) through the Uinta Formation to the top of the logged interval (46 ft bgs). <br />TM -4 (Attachment E) <br />CAST -M log completed: April 10, 2011 <br />Casing Depth: 1,907 ft bgs <br />Interval Logged: 46 — 1,827 ft bgs <br />As previously discussed cement was not observed at the ground surface during cementing operations at <br />TM -4. An attempt was made to cement the outside of the casing from the surface to correct the <br />deficiency. The volume of cement added was approximately equivalent to 230 feet of annulus space. The <br />CAST -M log was performed following the attempt to cement from the surface. <br />The ACE report prepared by Halliburton indicates the cement to pipe bond is adequate from the bottom of <br />the log (2,098 ft bgs; lower R3) to the upper R8 (696 ft bgs). The report indicates that the top of cement is <br />approximately 700 ft bgs. The bond is rated as good from the bottom of the log (lower R3) to the middle <br />of the R4 (1,644 ft bgs), fair from the middle of the R4 (1,644 ft bgs) to the upper R8 (696 ft bgs), and <br />
