6. A Financial Warranty was submitted for the project in the amount of $24,740.00. While <br />information about the hole sealing method and materials was provided, no documentation was <br />submitted to support the amount. Please provide the estimate and supporting documentation <br />used to establish the $24,740.00 amount. <br />7. The Division cannot calculate a reclamation liability estimate due to the insufficient information <br />provided in the NOI application at this time. <br />8. The Financial Warranty form submitted to the Division has two deficiencies within the form. <br />Please see the attached letter from the Division Main Office that outlines those deficiencies. <br />Communications with the Forest Service indicate that there are issues with the project that must be <br />addressed in their process. Please note that changes to the proposed project to meet the Forest Service <br />requirements may dictate changes to the Division NOI application that will require additional review. <br />This may lead to a request for additional information or deficiencies being noted by the Division. Per a <br />Memo of Understanding the Division and Forest Service must concur on financial warranty amount. A <br />process that cannot be completed until all information requested has been provided. <br />Based on communications from the Forest Service a site visit was conducted on July 13, 2011 with the <br />applicant. The Division was not notified and did not attend. Staff is electing not to visit the site at this <br />time. Once the deficiencies above are addressed a visit may or may not be required. <br />Per Rule 5.1.3(c), the Division is notifying the applicant of the above reference deficiencies.
