CQA ACTIVITIES: <br />SUMMARY: <br />ame <br />E) Geomembrane Delivery <br />1) VLF <br />Three loads consisting of 2 rolls of 40mil LLDPE and 33 rolls of 80mil LLDPE single sided <br />textured geomembrane was delivered to site during the week. The load was delivered on a flat <br />bed truck in good condition. The rolls were unloaded using a CAT 950 front -end loader with <br />forks attached. The geomembrane was placed on three pieces of HDPE pipe, stacked two rolls <br />high. An AMEC field representative inventoried the geomembrane and was present at all times <br />during unloading and stockpiling of the geomembrane. <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed slope contouring and subgrade compaction within the Phase 5 VLF. <br />AMEC inspected and approved subgrade prior to SLF placement within the Phase 5 VLF. <br />AMEC observed and assisted in the placement of SLF material within the Phase 5 VLF. <br />AMEC inspected and approved subgrade prior to geomembrane deployment within the Phase 5 VLF. <br />AMEC observed, logged data, and assisted in the installation of geomembrane within the Phase 5 VLF. <br />AMEC observed, obtained samples, and assisted in the process of DCF material at the DCF Process area. <br />AMEC observed the excavation of anchor trenches within the Phase 5 VLF. <br />AMEC observed anchor trench backfill within the Phase 5 VLF. <br />AMEC observed existing geomembrane exposure within the Phase 5 VLF. <br />AMEC observed the installation of the high volume solution collection system. <br />AMEC observed the DCF placement. <br />AMEC observed unloading and inventoried geomembrane delivered to site. <br />Testing Summary <br />Laboratory testing continued (Particle Size Distribution, Atterberg Limits, Moisture - Density Relations, <br />Specific Gravity, Permeability, and Point Load testing) for sampled materials. <br />VLF Weekly Report WE 07.02.2011 <br />
