Compliance With Mine Plan R - Support Facilities Not On-site <br /> R -Other NA -Special Categories Of Mining <br /> R -Processing Waste Y -Topsoil <br /> COMMENTS <br /> This was a complete inspection conducted by Janet Binns of CDRMS. The mine is a reclaimed underground <br /> mine, with a partially reclaimed load out located approximately 12 miles north west from the mine site. The <br /> area has been extremely dry with fire danger high. Grasses throughout the region have dried up. <br /> temperatures have been in the 90's already this growing season. <br /> The inspection started with a check of the records at the mine office in Florence. The inspector then visited <br /> the load out location,then progressed to the mine site. The Mine Access Road gate had been incorrectly <br /> locked by a third party,effectively locking all parties out of the property with the exception of those with <br /> access to the Vento lock key. Mr. George Patterson had met the inspector at the gate. Since the Vento lock <br /> bypassed the Energy Fuels locks,Mr.Patterson was unable to open the gate either. Mr. Patterson left the <br /> inspection at this point and the inspection continued on foot. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> The Mine records were checked in the Mine office in Florence. Availability of records sheet is enclosed with this <br /> inspection report. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> ❑Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7, <br /> 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br /> 4.05.13; Drainage—Acid and Toxic Materials 4.05.8;Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> Both ponds at the load out were dry. Embankments appear stable and no trash was dumped into either pond. <br /> Both ponds at the mine site were dry, embankments appeared stable and no problems were identified with either <br /> pond. The operator had inquired about a neighboring landowner routing runoff from the adjacent revoked mine to <br /> pond 5. The Division is uncomfortable with this proposal as long as the pond is required to treat runoff from the <br /> Southfield mine.
