3. Permit page 2.05 -7 contains a discussion of the drilling activities and planned future <br />monitoring of wells 94 -001 and 95 -14. Based on a review of current drill holes and <br />hydrological monitoring sites, the status of these wells is unclear. Hole 95 -14 is shown <br />on Map 11 — Hydrologic Monitoring Program Location Map but it is not sampled, <br />included in the Annual Hydrology Report or shown on AHR Figure 2 — Hydrologic <br />Monitoring Program Location Map. The Division could find no other reference to hole <br />94 -001. Please review the information on page 2.05 -7 for holes 94 -001 and 95 -14 and <br />update their status as necessary. Please provide the Division with a discussion of <br />proposed well 95 -14 and its possible inclusion in the hydrologic monitoring plan. <br />4. Permit page 2.05 -9 discusses the Multiple Services Building at the Mine 5A portals. <br />During 2010 and early 2011 MCM has removed oil tanks and barrels of waste oil and <br />other products. Please update the status and reclamation activities of the Multiple <br />Services Building on permit page 2.05 -9. <br />5. Permit page 2.05 -12 discusses the "Mine Water Handling System ", which pumps water <br />from the No. 5 mine to the surface (for discharge through pond 5P -7 to the Williams Fork <br />River). In late 2010 the mine pump went down and was scheduled to be replaced late in <br />the spring of 2011. The new pump may change the pumping rate /volume from the Mine <br />No. 5 workings. Please review the pumping information contained in the "Mine <br />Water Handling System" section, beginning on permit page 2.05 -12, to ensure that it <br />provides the correct information for the newly installed pump. <br />6. Permit page 2.05 -15 has a table that provides a listing of all of the petroleum products <br />stored on the site. Given the recent reclamation at the site, particularly at the Multiple <br />Services Building, the information contained in this table is out of date. Please review <br />the Petroleum Product Storage table beginning on permit page 2.05 -15 and update <br />it as necessary based on the recent reclamation at the site. <br />7. Permit page 2.05 -16 has a section titled "Waste Disposal Sites ". This section contains <br />information regarding the current refuse pile at the Williams Fork Mines. The section <br />discusses the "almost complete" refuse pile at the No. 9 Mine site. This refuse pile was, <br />however, reclaimed in 2009. Please update permit page 2.05 -16 to reflect the current <br />reclamation status of the refuse pile at the No. 9 Mine site. Please include a <br />comment verifying that the required four feet of cover was placed on the pile <br />(permit page 2.05 -29). <br />8. Permit page 2.05 -16 has a section titled "Waste Disposal Sites ". This section contains <br />information regarding the current refuse pile at the Williams Fork Mines. There is also a <br />solid waste disposal site that was permitted in November 2007 (with TR -34) that should <br />be included in this section. Please add a description of the solid waste disposal site <br />that was permitted with TR -34 in November 2007 to the Waste Disposal Sites <br />section on permit page 2.05 -16.
