• <br />amec <br />Earth & Environmental <br />V) VLF Geosynthetics <br />Geomembrane installation within the Phase 5 VLF continued throughout the week, performing <br />deployment, seaming, and repair /QC activities. Geomembrane installation was performed in the area <br />shown in Figure 4. Approximately 208,525 square feet of 100mil LLDPE single sided textured was <br />deployed using a John Deere 200 excavator and a John Deere 544 front -end loader with spreader -bar <br />attachments. Subgrade was inspected and approved by AMEC and Lonestar representatives prior to <br />geomembrane deployment (see Figure 4). Approximately 11,523 linear feet of seam was fusion <br />welded during deployment using three fusion welding machines. Twenty -two destructive test samples <br />were marked during seaming. All destructive sample test results met project specifications. Non- <br />destructive testing was performed for all fusion welded seams. Repairs were performed using the <br />extrusion welding method for all marked defects. Vacuum testing was performed for all extrusion <br />welded repairs. <br />Panels S21 through S114 (secondary- 100mil) were inspected by AMEC and Lonestar representatives, <br />accepting the area (see Figure 5). <br />One load (11 rolls)of 100mil LLDPE single sided textured geomembrane, one load (12 rolls) of <br />100mil smooth geomembrane, and three loads (33 rolls) of 80mil LLDPE single sided textured <br />geomembrane were delivered to site during the week for a total of 56 rolls. All loads were delivered <br />on flat bed trucks in good condition. The rolls were unloaded using a CAT 966 front -end loader with <br />forks attached. The geomembrane was placed on three pieces of 4" HDPE pipe, staked two rolls high. <br />An AMEC field representative was present at all times during unloading of the geomembrane. The <br />AMEC representative also inventoried the geomembrane, as it was unloaded. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed structural fill placement within the Phase 5 VLF fill area. Ambient temperatures were <br />monitored during fill placement. <br />AMEC observed, performed necessary testing, obtained moisture samples, and assisted in the placement <br />and moisture conditioning of SLF material within the Phase 5 VLF. <br />AMEC inspected and approved subgrade for geomembrane deployment within the Phase 5 VLF. <br />AMEC observed, logged data, and assisted in the installation of geomembrane within the Phase 5 VLF. <br />AMEC inspected and accepted secondary geomembrane within the Phase 5 VLF. <br />AMEC observed construction of the PSSA ore slope constantly. <br />VLF Weekly Report WE 06.05.10.D <br />
