-'F <br />ameO <br />VLF EARTHWORKS FIELD MONITORING SUMMARY (WEEKLY REPORT) <br />Project Title: CC&V/ Phase 5 <br />Owner: Cripple Creek & Victor Gold Mining Co. <br />Location: Cripple Creek & Victor Gold Mine <br />Work Element: Phase 5 CQA/ VLF <br />Project No.: 84201125EO <br />Contractor: Ames Construction Co., Inc. <br />Shift: Day <br />Date: May 21, 2011 <br />Reporting Period: 5/15/11 through 5/21/11 <br />Days S M T WI T F S <br />Work Shifts - D D D D D D <br /> <br />D = Day Shift N = Night Shift <br />Ambient Temperature Range (F): Low: 29°F to 37°F (+/-) High: 44°F to 66°F (+/-) <br />AMEC CQA: Personnel On-site through the Reporting Period (See Attachment A). <br />AMES: Rain and snow showers hindered work activities throughout the week. <br />CONSTRUCTION ACTIVITIES and PROGRESS: <br />I) EARTHWORKS <br />A) Subgrade Preparation <br />1) VLF <br />A CAT 563 vibratory smooth drum compactor was used to compact subgrade between stations <br />J0+00 (+/-) and J7+00 (+/-). The subgrade surface was moisture conditioned (when necessary) <br />and compacted with at least 4-passes. <br />B) Slope Contouring <br />1) VLF <br />Slope contouring performed between stations J0+00 (+/-) and J15+00 (+/-), approximately 600 <br />feet left of centerline to 300 feet right of centerline and between stations H14+00 (+/-) and <br />H24+00 (+/-), approximately 0 to 300 feet left and right of centerline using two CAT D-6 LGP <br />dozers and two CAT D-8 dozers. Excess material from slope contouring was loaded into 40- <br />ton articulating haul trucks using a CAT 950 front-end loader and was hauled to an area <br />designated by CC&V. <br />C) Cut to Fill <br />1) VLF <br />Structural fill was placed between stations J 10+00 (+/-) and J 10+75 (+/-), approximately 200 <br />to 250 feet left of centerline. Structural fill was placed in 18" to 24" lifts using various CAT <br />dozes. Each lift was moisture conditioned using a tandem axle water truck (when necessary) <br />and compacted using a 10-ton vibratory smooth drum compactor, passing over the entire fill <br />VLF Weekly Report WE 05.21.2011
