amec'0 <br />VLF EARTHWORKS FIELD MONITORING SUMMARY (WEEKLY REPORT) <br />Project Title: CC&V/ Phase 5 <br />Owner: Cripple Creek & Victor Gold Mining Co. <br />Location: Cripple Creek & Victor Gold Mine <br />Work Element: Phase 5 CQA/ VLF <br />Project No.: 84201125EO <br />Contractor: Ames Construction Co., Inc. <br />Shift: Day <br />Date: May 14, 2011 <br />Reporting Period: 5/8/11 through 5/14/11 <br />Days S M T WI T F S <br />Work Shifts - D D D D D <br /> L <br />D = Day Shift N = Night Shift <br />Ambient Temperature Range (F): Low: 27°F to 39°F (+/-) High: 41°F to 62°F (+/-) <br />AMEC CQA: Personnel On-site through the Reporting Period (See Attachment A). <br />AMES: Ames paused work activities at approximately 12:00pm on Wednesday (5/11/2011) due to <br />slick/saturated haul routes, from snowfall. Work activities were not performed for the remainder of the <br />week. <br />CONSTRUCTION ACTIVITIES and PROGRESS: <br />I) EARTHWORKS <br />A) Subgrade Preparation <br />1) VLF <br />A CAT 563 vibratory smooth drum compactor was used to compact subgrade between stations <br />J7+00 (+/-) and J13+00 (+/-). The subgrade surface was moisture conditioned (when <br />necessary) and compacted with at least 4-passes. <br />Subgrade preparation was performed between stations J 10+00 (+/-) and J10+75 (+/-), <br />approximately 200 to 250 feet left of centerline. A CAT D-8 dozer was used to rip the <br />subgrade surface to a depth of 6" to 8". The subgrade was then moisture conditioned using a <br />CAT 773 water tuck (when necessary) and compacted using a 10-ton vibratory smooth drum <br />compactor. The entire surface of the subgrade was compacted with at least 4-passes. This area <br />was inspected by an AMEC representative and accepted for structural fill placement. Soil <br />temperatures were monitored prior to structural placement and found to be acceptable. <br />B) Slope Contouring <br />1) VLF <br />Slope contouring performed between stations J0+00 (+/-) and J24+00 (+/-), approximately 600 <br />feet left of centerline to 300 feet right of centerline and between stations H8+00 (+/-) and <br />VLF Weekly Report WE 05.14.2011
