Access Road Summary table will be prepared and evaluated, a report will be submitted to DRMS and the following additional reporting will be adopted: o Movement > 0.2 feet between consecutive readings: immediately evaluate cause of movement, take corrective action as necessary, and report to County and DRMS; o Movement > 0.4 feet between consecutive readings: use of road will immediately cease and the cause of movement evaluated, corrective action taken as necessary; and results reported to County and DRMS. Inspection and Maintenance Program On monthly basis: • Remove debris, slash, and trees to a temporary storage area located at 22+40 • All paper, tape, wood lathes etc will be bagged and disposed in an offsite landfill • Inspect retaining wall, roadway for signs of distress • Check for debris in roadway, remove as necessary • Check condition of silt fence at base of slope; remove soil/rock buildup and repair any damage observed as necessary • Visually Check and record cracks in roadway; Reporting: • Prior to work activity obtain photographs of the project site; • Document daily work activities such as: o Environmental engineering construction monitoring records; o Clear area and dispose debris, trash, and paper offsite; o Remove trees, slash and properly store materials in designated areas; • If required, obtain DRMS signatures on daily work sheets; • Prepare and submit "as-built" drawings; • "As built" drawings will be prepared in the third quarter of 2011 where center lines, cuts and fills have been established; • Survey monuments will be placed and located on a base map to monitor road fill stability; • Monthly monitoring reports will be submitted quarterly (unless movement is recorded) to County and DRMS within 10 days of receipt of results from surveyor; and • Corrective action reports, summarizing the results of corrective action taken if allowable movement thresholds exceeded.
