Response to CN-01 Third Adequacy Issues - June 6, 2011 <br />Response #2 <br />Proof of Notice to Owners of Record of the surface and mineral rights of the affected land <br />and the Owners of Record of all land surfaces within 200 feet of the affected land <br />boundary, required by Rules 1 6 2(1)(e)(i) and (ii) is provided in Attachment B <br />Individuals/entities within 200 foot of the affected land boundary include the following: <br />Ken and Verniece Fa erlin David and Erin Linden <br />David N. Huckaba Liberty Communication <br />Kelly Okamura <br />Mans - Rules 6.2.1(2). 6.4.3. 6.4.4, and 6 4 6 <br />Adequacy Issue #3 <br />Pursuant to Rule 6.2.1(2)(d), all maps of the application must identify and outline the permit <br />boundary and the boundary of affected lands. There are two boundaries which must be <br />delineated and labeled on the maps; the permit boundary, enclosing approximately 274.4 acres, <br />and the boundary of affected lands, enclosing approximately 47.6 acres. All affected lands must <br />be enclosed within the boundary of affected lands and all affected lands must be enclosed within <br />the permit boundary. <br />a. Please revise the maps of the application to delineate and label the correct permit boundary, <br />which now includes the one-acre staging yard. <br />b. Please revise the maps of the application to delineate and label the correct boundary of <br />affected lands. The boundary of affected lands shall enclose the following areas: <br />• May Day 1- 3.5 acres; <br />• May Day 1 East - 0.5 acres; <br />• May Day 2 - 3.3 acres; <br />• May Day 3 (Lamb) - 3.2 acres; <br />• Idaho Millsite - 3.1 acres; <br />• Pay Day Millsite - 5.1 acres; <br />• Augmentation Pipeline Corridor - 0.4 acres; <br />• Chief Tunnel - 0.3 acres; <br /> <br />Revised <br />6/6/2011
