Fourth Adequacy Response 112(d)-1 Application, Amended CN-01 <br />Wildcat Mining Corporation May Day Idaho Mine Complex <br />Attachment A <br />Portal Repair Adequacy Response <br />Fourth Review Adequacy Response <br />May 25, 2011 Cazier Memorandum <br />Permit No.1981-185/CN-O1 <br />The following comment are posed by Mr. Cazier to ensure adequate engineering analyses <br />and design practices are implemented in accordance with Rule 6.4.21 (Designated Mining <br />Operation Environmental Protection Plan). His memo follows the Company's response. <br />Response to General Comments <br />A. May Day No. 1 Portal Collapse Repair work plan is presented in Attachment I to <br />the Third Adequacy Response (June 6, 2011); <br />B. May Day No. 1 Steel set stability analysis is presented in Attachment P (Exhibit 6.5) <br />to the Third Adequacy Response (June 6, 2011); <br />C. May Day No. 1 Portal Collapse Repair work plan is presented in Attachment I to <br />the Third Adequacy Response (June 6, 2011); Work plan for the May Day No. 1 <br />drainage is presented in the Stormwater Management Plan (Attachment D-1) and <br />Attachment K (Chief Little Deadwood Gulch Corrective Action Plan) to the Third <br />Adequacy Response (June 6, 2011); <br />Attachment E-8, 6.4 Geotechnical Stability: <br />1. Slopes steeper than 1.50(H):1(V) that are disturbed by mining activity would be <br />reviewed for stability. All slopes greater than 1.5(H):1(V) have been field evaluated <br />for stability concerns by a registered professional geotechnical engineer (Mr. Erich <br />Rauber-Colorado PE #26647-30 years experience). The Company commits to <br />providing this additional geotechnical analysis regarding disturbed slopes greater <br />than 1.5(H):1(V) as soon as it is able to obtain access across the U.S. Forest Service <br />road or the new access road with necessary equipment. <br />2. See the Company's response to Mr. Cazier's March 11, 2011 Memorandum <br />submitted contemporaneously herewith on June 6, 2011. <br />3. DRMS is correct the word "unrelated" should have read "related." <br />4. Steel foundation stability preparation will be proposed when the foundation within <br />collapsed portal has been cleared of debris. <br />5. The Company understands the highwall associated with the May Day No. 1 Collapse <br />Portal must be geotechnically and geologically assessed prior to commencing repair <br />work. <br />6/6/2011 <br />9
