CQA ACTIVITIES:
SUMMARY:
The CQA activities performed consisted of observing, documenting and recording the construction
activities of the contractor.
AMEC observed slope contouring of the Phase 5 VLF slopes.
AMEC observed and inventoried geomembrane delivered to site.
Testing Summary
Laboratory testing continued (Particle Size Distribution, Atterberg Limits, Moisture-Density Relations,
Specific Gravity, Permeability, and Point Load testing) for sampled materials.

CC&V held a Pre-construction meeting on April 12, 2011. Items discussed included Safety,
Environmental, Schedule, QC documentation, and CQA.
Jay Moore (AMEC) and Kim Morrison (AMEC) were onsite to attend the Pre-construction meeting and to
observe project activities.
AMEC personnel performed an inspection of the Phase 5 construction activities with Timm Commer
(CC&V) and Berhan Keffelew (DRMS). No issues were encountered during the inspection.
