Furthermore, all maps must include the following <br />elements: <br />a) Show the name of the Applicant. <br />b) Must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person. <br />c) Give date prepared. <br />d) Identify and outline the area which corresponds with the application <br />e) The acceptable range of map scales shall not be larger than 1 inch = 50 feet nor smaller <br />than 1 inch = 660 feet. Also, include a map scale, appropriate legend, map title and a <br />north arrow. <br />6..4.12 Exhibit L - Reclamation Costs <br />10. Please provide the haul or push distance from the concrete rubble pile to the backfill location. <br />11. Please provide volume and unit costs for topsoil application and backfilling in terms of cost <br />per cubic yard. <br />12. The Operator has estimated 65.36 acres will require seed and mulch application. The <br />Headley Mine is permitted for a total of 59.3 acres. Please clarify this discrepancy and make <br />any changes to the reclamation cost estimate as necessary. <br />This concludes the Division's preliminary adequacy review of this application. Please remember <br />that the decision date for this application is June 1, 2011. If you are unable to provide <br />satisfactory responses to any inadequacies prior to this date, it will be your responsibility to <br />request an extension of time to allow for continued review of this application.
