MARCH 24, 2011 <br />MAY DAY MINE - SUBMITTAL 1 REVIEW <br />PAGE 4 OF 5 <br />Colorado Department of Public Health and Environment Water Quality Control Division (CDPHE <br />WQCD) and Environmental Protection Agency (EPA) reviews in anticipation that water quality will be <br />thoroughly addressed within those established processes. <br />Once these items are reviewed, the County may have additional comments or requests related to water <br />quantity and water quality. <br />III. Submittals Needed Prior to Further Consideration <br />A. Narrative addressing <br />1. Mining process, location, and associated improvements (summary-level only) <br />2. Water supply (summary level only) <br />3. Traffic projected on CR 124 and on the project access roads - including current existing traffic and <br />proposed project traffic <br />4. Access to the site - road grades, turn radius, design vehicles and loads, acceptable uses <br />5. Number of employees, access to site, and parking <br />6. Service vehicles anticipated and access to site <br />7. Sewer service at the site <br />8. Potable water service at the site <br />9. Solid waste removal <br />10. Emergency service at the site <br />11. Snow management (Will the mine operate seasonally and, if not, how will employees and services <br />access the site in snow conditions? Where will snow be stored?) <br />B. Mine Rescue Plan if road grades are not established to allow for fire district access <br />C. Scaled Site Plan at a scale of 1"=100' on 24" x 36' sheets addressing: <br />1. Full mine site from CR 124 access point to mine locations <br />2. Location of property lines <br />3. Location, width, and reception numbers for easements (utilities, access, railroad, etc.) <br />4. Location of existing utilities <br />5. Location of existing and proposed improvements including parking, key mining operations, <br />buildings, material storage areas, snow storage areas, etc <br />6. Location of all project water sources, storage areas, augmentation ponds, transmission lines, and <br />fire-fighting water storage <br />7. Proposed drainage including rough topographic contours <br />D. Engineered road plans for the new and unpermitted portion of the access road west of and crossing the <br />La Plata River. The Plans shall include detailed topography, road plans and profile, road cross- <br />sections, drainage, material specifications, structural specifications, materials and compaction testing, <br />and other critical design components.
