Idaho Mine and Millsite, Wildcat Mining Corporation, Original 112(d) Permit <br />Exhibit T - Permanent Man-Made Structures <br />Fairview Land Corporation -driveway access, access roads (Little Deadwood Jeep <br />i Trail), and the La Plata bridge <br />La Plata County-Public Works-Count Road 124 <br />United States Forest Service-Columbine District-Land Managers <br />With the exception of the Taylor's well, all other listed domestic wells are completed in the <br />La Plata river alluvium. In addition, the State Engineers Records suggest well (permit <br />number 64335) was completed within the Wildcat Mining Corporation permit boundary. <br />An extensive field survey could not locate the well and is deemed to be abandoned or the <br />well coordinates were not accurately reported to the State Engineer. <br />Returned signed agreements were obtained from Mr and Mrs A Taylor, Owest and La <br />Plata Electric. Under the agreement, fences, wells, utilities, etc., will be repaired or <br />replaced if impacted by Wildcat Mining <br />Wildcat Mining does not believe the wells are "valuable" under the Mining Rules Wildcat <br />Mining estimates these wells can be replaced for less than one or two thousand dollars <br />each. Nonetheless, Wildcat Mining has submitted engineering and hydrogeologic <br />evaluations by registered professional engineers and a Wrogeologist who have determined <br />that impacts to manmade structures within 200 feet of the permit area are not reasonably <br />anticipated. <br />In response to the concerns raised by Mr. Linden regarding the transportation of chemicals <br />and fuels near his well, Wildcat Mining has also committed to locate emergency response <br />kits at the entrance of CR-124 and in close proximity to the iuncture of the USFS road and <br />the access road. The emergency response kits will include absorbents, booms, a 55 gallon <br />drum and a fire extinguisher. <br />The purpose of the Access Easement is for vehicular access and the establishment of a base of <br />operation. (See Exhibit D for more details) The access road and base of operation is considered no <br />risk to damaging the domestic wells located well away from the road <br />Vehicles of less than 4-5--25-ton only will use the bridge crossing the La Plata River. As the upgraded <br />bridge is rated at 4125-ton carrying capacity, there is considered no risk to damaging the bridge. <br />Vehicular traffic will be restricted to the road itself and will not affect the boundary fences. <br />May Day Idaho Mine Complex 112(d) Permit Application <br />Revised: April 14, 2011 <br />2
