Pineda <br />Director <br />The amendment application for your gravel pit, referenced above, was received here at the Division's Durango field <br />office on March 18, 2011. Our initial review of the application reveals that it is lacking some information, which <br />makes the application "incomplete." This letter lists the items needed to complete the application. Until these items <br />are submitted, we cannot process the application further. Please provide each of the items listed in the following <br />paragraphs. You should review the submittal requirements contained in the Construction Materials Rules, cited <br />below, if necessary: <br />Exhibit E, Mininja and Reclamation Plan Maps, Rule 6.3.5 <br />There should be at least two maps, so that features involving the mining activities and mining plan may be shown <br />separately from those involving the reclamation. Maps must be signed and dated by a qualified person who prepared <br />the maps. Additional map requirements are described in Rule 6.2.1(2). <br />Exhibit F, List of Other Permits and Licenses, Rule 6.3.6 <br />This should identify any county or local district permits that may be required, as well as any state permits, such as <br />from CDPHE for air emissions or stormwater controls. If there are no other licenses or permits needed, simply state <br />this. <br />Exhibit H, Municipalities within a Two-Mile Radius, Rule 6.3.8 <br />This has not changed since the initial permit application, so you may reference the original 1988 application, or you <br />may simply state there is no municipality within two miles. <br />Exhibit I, Proof of Filing Application with County Clerk and Recorder, Rule 6.3.9 <br />This exhibit was properly included in the initial amendment materials. However, please note that the amendment <br />application materials on file with the county clerk must be kept current with your submittals to DRMS.
