2.05 APPLICATION FOR PERMIT FOR SURFACE OR UNDERGROUND MINING ACTIVITIES -
MINLMUM REQUIREMENTS FOR OPERATION AND RECLAMATION PLANS.
(D) Not Applicable - no mining will occur or has
occurred under these structures.
(E) Not Applicable - all information required is
included within this Mine Plan.
(ii) Design Plans
(A) The maps, plans and cross sections for the East Mine
ponds, impoundments and diversions required under
Colorado Rules and Regulations were prepared by or
under the direction of and certified by Matthew
Sakurada, P.E., a qualified professional engineer
with engineering experience in the design and
construction of sedimentation ponds, excess spoil
and underground development waste facilities. Mr.
Sakurada holds a B.S. and M.E. in Civil Engineering
_ and is registered in the State of Colorado, P.E.
Number 15836. Mr. Sakurada was assisted by Charles
Leaf, PhD., P.E., a consulting hydrologic engineer,
and Geo-Hydro Consulting, Inc.
The West Mine maps, plans and cross-sections
required under Colorado rules and regulations wre
prepared by or under the direction of Graeme Major
P.E., a consulting engineer with Golder Associates.
The construction of the sedimentation ponds was
supervised and inspected by Matthew Sakurada, P.E.

Volume SA and Waste Disposal Appendix in Volume 6A.
Also see the Orchard Valley West Portals designs in
Volume 6.
(C) See the~POnds, Impoundments, and Dams Appendix in
