<br />Task 11- Provide a Polyurethane foam plug, if necessary
<br />This item is to provide a PUF enclosure in the mine portal should it be necessary for a safe
<br />closure once the portal opening is uncovered. This item includes all labor, equipment, material,
<br />and support costs to provide 10 cubic yards of PUF. It will be reimbursed on a cubic yard of
<br />PUF placed.
<br />Upon award of the contract, J3 is prepared to enter into an agreement with the State, and has no
<br />issues with the sample purchase order language provided in the bid package. In addition, as
<br />part of contract submittals, J3 will prepare and submit the following items to the State:
<br />A site-specific Safety, Health, and Environmental Action Plan (SHEAP) that will address all
<br />of the required elements as indicated in the bid documents, including but not limited to
<br />personal protective equipment (safety glasses, reflective traffic vests, steel-toed boots, and
<br />hard hats, safety harnesses and lanyards), first aid kits and supplies, fire protection
<br />equipment, a fire safety procedure, job safety analysis sheets detailing the hazards of the
<br />work as well as specific to each task, a training procedure to the plan and JSAs, maps to the
<br />nearest medical facility, accident prevention and reporting procedures, on-site hand and
<br />radio communications, site access procedures, the J3 substance abuse policy, a project
<br />organizational chart detailing the roles of each key person such as the Health and Safety
<br />representative, daily tailgate meeting record logs, equipment inspection logs and
<br />procedures, elevated area work procedures, procedures around hazardous mine openings,
<br />procedures for handling all generated wastes - both liquids and solids, and hot work
<br />procedures, as necessary.
<br />- Surety - provide the necessary surety required for execution of the agreement and
<br />performance of this project. In this case, it appears that performance, labor and material
<br />payment bonds will be necessary for this project, according to the bid documentation and
<br />sample purchase order terms and conditions.
<br />- Certificates of Insurance - showing proof of required coverage, including fire protection, and
<br />naming the State as an additional insured.
<br />AML Contractor Ownership and Control Form - we will complete this form and submit with
<br />the contract documents as required.
<br />Minority (MBE) and Women (WBE) Owned Business Participation Form - we will complete
<br />this form and submit with the contract documents, as required.
<br />Construction Project Schedule - we anticipate the work to take approximately 10 working
<br />days and propose to work, from 7:00 am to 5:00 pm, Monday through Saturday to
<br />accomplish the scope of work. J3 is prepared to start work activities by February 23, 2009 to
<br />allow for completion within 19 days of the Notice to Proceed, including mobilization and
<br />demobilization. We anticipate completion of the above scope of work in accordance with the
<br />project schedule, on or about March 13, 2009.
<br />Subcontractor List - J3 will self-perform most of the scope of work, with the exception of the
<br />hauling of equipment to the project site. That work will be performed by the equipment
<br />vendor.
