The report shall <br />include an accounting of all wastewater plant water effluent controlled by each entity, <br />showing the total volume of water under its control at the various wastewater treatment <br />plants, and the amount committed to each of the recipients of the water. The applicant <br />must submit the required reports to the water commissioners on a monthly basis. If <br />the applicant does not submit the required reports the SWSP cannot be administered <br />and credit for the released effluent will not be granted. <br />12. All releases of replacement water must be sufficient to cover all out-of-priority depletions in <br />time, place, and amount and must be made under the direction and/or the approval of the <br />water commissioner(s). The release of replacement water may be aggregated to maximize <br />beneficial use. The water commissioner(s) and/or the division engineer shall determine the <br />rate and timing of an aggregated release. <br />13. The approval of this substitute water supply plan does not relieve the Applicant <br />and/or landowner of the requirement to obtain a Water Court decree approving a <br />permanent plan for augmentation or mitigation to ensure the permanent replacement <br />of all depletions, including long-term evaporation losses and lagged depletions after <br />gravel mining operations have ceased. Therefore, the applicant is required to file for <br />a permanent plan for augmentation with the Water Court by the expiration date <br />(December 31, 2011) of this substitute water supply plan to include the long-term <br />evaporation losses at the Distel Pit and the F Street Pit. <br />14. The replacement water, which is the subject of this plan cannot be sold or leased to any <br />other entity. As a condition of subsequent renewals of this substitute water supply plan, the <br />replacement water must be appurtenant to this site until a plan for augmentation is <br />obtained. All replacement water must be concurrent with depletions in quantity, timing, and <br />locations. According to an affidavit dated January 25, 2001, the Applicant dedicated the <br />12.5 shares of the Whitney Ditch Company and 96 shares of the Fulton Irrigating Ditch <br />Company as replacement water solely for this SWSP for as long as there are depletions at <br />these gravel pit sites or until such time as another replacement source is obtained. For the <br />purposes of this SWSP, this affidavit will be accepted for the dedication of the <br />shares; however, if the State Engineer determines that a different affidavit or <br />dedication process is necessary to assure proper dedication of the shares, additional <br />information may be required prior to future SWSP approvals. <br />15. The Applicant shall perform an inspection and provide verification for all parcels of dried up <br />land used to generate augmentation credits during the term of this SWSP. The final <br />verification of dry up will be in the form of an affidavit signed by an individual having <br />personal knowledge of the dry up for the entire irrigation season for each parcel of land <br />used in this SWSP. In accordance with the attached protocol for dry-up of irrigated land, <br />the Applicant shall provide a written notification to the water commissioner and division <br />engineer by April 1, 2011 identifying the lands to be dried up for the 2011 irrigation season. <br />By October 15, 2011 the Applicant shall provide an affidavit to the water commissioner and <br />division engineer that identifies and confirms the lands that were dried up during the 2011 <br />irrigation season in order that the final determination of augmentation credits for the <br />irrigation season can be made along with mapping showing any revisions to the dried-up <br />acreage. The Applicant shall modify accounting to reflect that the credit from any dried up <br />fields containing alfalfa or native grass was assessed in the following manner:
