PERMIT #: M-2002-108 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: March 16, 2011 <br />CORRECTIVE ACTIONS: The operator shall seed the stockpiles with the seed mix that was submitted as part of <br />the approved Reclamation Plan. If no seed mix has been provided, the operator shall submit a seed mix for <br />approval as a technical revision to the permit along with the appropriate $216 revision fee to the Division by the'. <br />corrective action date. The Operator shall demonstrate compliance by submitting seed tags, a bill of sale or <br />photographs of seeding activities. The Operator may also request a follow up inspection. <br />CORRECTIVE ACTION DUE DATE: 4/15/11 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Colorado Division of Reclamation, Mining and <br />Safety (Division). Kenneth Morrison of Lincoln County was also present for the inspection. The Eikermann Site <br />is located approximately 11 miles east of Hugo, Colorado. The site is permitted for 9.5 acres and the post- <br />mining land use is rangeland. <br />Gen. Compliance With Mine Plan: <br />The Eikermann site is operated less than 180 days per year; no mining was taking place during the inspection. <br />According to the 2010 Annual Report, approximately 5 acres of the permit area has been disturbed. No <br />reclamation has been performed to date. Two large piles of ground asphalt have been placed on the pit floor. <br />According to Mr. Morrison, the material originated from a Lincoln County road project. The approved Mining <br />Plan does not allow for the storage of this material at the site. The presence of these asphalt piles has been <br />cited as a problem and will require corrective action by the Operator. <br />Signs and Markers: <br />The permit sign was posted at the site entrance and the boundary of the affected area was clearly delineated <br />pursuant to Construction Materials Rule 3.1.12. <br />Topsoil: <br />Topsoil has been stockpiled around the perimeter of the site. The topsoil stockpiles are stable; however, they <br />have not been seeded with the approved seed mix. This has been cited as a problem and will require <br />corrective action by the Operator. <br />This concluded the inspection. <br />Page 2 of 4 <br />
