On Figure 2.2-5, the naming of the haul roads is inconsistent with the application text. <br />Please rename the Portal Haul Road "Haul Road #1" and the Gob Pile Haul Road "Haul <br />Road #2". <br />3. Figure 2.2-5 provides cross sections for three specific stations for Haul Road #1. Please <br />revise this map to also include cross sections for Haul Road #2 at approximate stations 6+00, <br />13+50 and 21+00. <br />4. Access Road #1 is described at the top of page 2.05-18. The description includes length and <br />grade, but the width is not given. Please revise the description to give the width of proposed <br />Access Road #1. <br />5. No plans and drawings specific to Access Road #1 have been submitted, as required by <br />2.05.2(3)(c)(vii). Please provide plan and profile information for the access road which <br />demonstrates that the design criteria of 4.03.2 have been met. <br />6. Page 2.05-16 describes the Design and Construction of roads on the site. The third paragraph <br />of the section addresses embankment specifications for road sections not within the disturbed <br />area in accordance with Rule 4.05.2(4). Please provide a description of (or illustrate <br />graphically on Figure 2.2-5 Haul Road Plan and Profile) the specific road segments to which <br />these specifications will be applied. <br />1. Paragraph (ii)(B) on page 2.05-21 references Rule 4.05.6(11):a. This reference appears to be <br />in error. The Division believes that the intended reference may be Rule 4.05.9(7). Please <br />review, and revise as necessary. <br />2. Rule 4.05.9(7)(d) requires that the minimum elevation of the top of the settled embankment <br />be 1.0 foot above the water surface in the pond with the emergency spillway flowing at <br />design depth. Please revise the third paragraph on page 2.05-22 to include the specific <br />language of (7)(d). <br />3. Surface water control systems are depicted on Figure 2.2-3. The Division has the following <br />comments regarding this map. <br />a) Three cross sections of the pond feature are provided at horizontal scales of 1" = 10' and <br />I" = 30'. The 1" =200' plan view of the proposed sedimentation pond does not provide <br />sufficient detail for review. Please revise Figure 2.2-3 to include a detailed plan view of <br />the proposed sedimentation pond at a scale of 1" = 30' or 1" = 10'. <br />b) On Section A-A', please label the outslope (h:v) of the pond embankment <br />4. The requirements of 4.05.9(14), (15) and (17) regarding inspection of the impoundment do <br />not appear to have been addressed in the application. Please provide commitments in the <br />application regarding inspection and certified inspection reports for construction of the <br />sedimentation pond, and quarterly inspections, in accordance with these sections. <br />2.05.3(5) and 4.06 Topsoil <br />1. The fifth paragraph on page 2.05-26 states that the proposed topsoil stockpile will have a <br />maximum vertical height of 50 feet, with side slopes constructed at 2.5h:1v. However, <br />Figure 2.2-2 shows 2.0h: Iv for the side slopes. Please review and revise as necessary for <br />consistency. <br />
